These fluoride filters appear to not have a stem. How do they attach?

They actually attach to the thread on the carbon filter. They sit in the water that’s already filtered through the tank. So you’d put the carbon filter in first then with 8 (they’re very specific about this 8 turns only business) turns you attach the fluoride filter and you’re ready to go. How long should these last?

Truly it depends on your water and how much you use it. We (family of 3) generally go through roughly a gallon a day of water. We have 4 of these in our big berkey and we probably replace them at two years, give or take 6 months. We wash the unit and let all filters dry out ever 3 months or so. Technically speaking they're supposed to be replaced after 1000 gallons (per filter).

Will this filter chlorine?

Yes. That's the primary reason we use it.
