Does this work for the inside skin of fruit?

It will only work on the outside skin (assuming you're making twists). And the outside skin must be fairly think: This means it will, for instance, work on a regular lime--but not on a key lime.

This tool looks like it should create several thin 'curls' (zest only) if you use the small holes at the end, or a single thicker strip (with some 'pith' attached) if you use the channel in the middle. In my experience, it wasn't sharp enough to do either. The thin curls never worked, and the channel took an enormous amount of strength and was so thick that it did not curl! I tried this tool with Lisbon (non-Meyer) lemons and Robertson (navel) oranges since those are the varieties we grow. I donated mine to a charity.

does this work for a left-handed cook?

I’m right handed but I think it would work for you if left handed. It’s the same on both sides.

Is this the right tool to make the flower like shapes of sliced carrots and cucumber slices that come in stir fry's or as a garnish in Thai food?

No. This is a tool for taking long strips of zest from citrus fruit. Most of the flower shapes are cut by hand with a very small, sharp paring knife.
